We collect personal information you give us, such as your name, contact details, date of birth, emergency contact, Medicare or referral details, and the health information needed to provide psychological care. We usually collect this directly from you, and sometimes from your GP or referring practitioner with your consent.

❋ Why we collect it

To provide psychological assessment and treatment, manage appointments and payments, communicate with you and (with your consent) your other treating practitioners, and meet our legal and professional obligations.

❋ How we store and protect it

Your information is held in our secure practice management system (Halaxy) and in secure clinical records. We take reasonable steps to protect it from misuse, loss, and unauthorised access, and we keep records for the period required by law.

❋ Access and correction

You can ask to access or correct your information at any time by contacting us. We will respond in line with the APPs.

❋ Who we disclose it to

We do not sell your information. We may disclose it with your consent (for example to your GP), where required or authorised by law, or where there is a serious risk to your safety or the safety of others. Some of our providers (for example our practice software and website host) store data on our behalf, and some may store data overseas.
